Idiopathic pulmonary fibrosis (IPF) is the most common type of idiopathic ILD, and represents, by definition, the prototype of progressive fibrosing ILD (PF-ILD) (Raghu et al. 2011). It is characterized by a self-sustaining progressive fibrosis, worsening respiratory symptoms, declining lung function, worsening quality of life, and early death despite clinical management. Clinical data suggest that there is a larger group of patients with differing clinical ILD diagnoses (of known and unknown cause) who develop a progressive fibrosing phenotype during the course of their disease (Wuyts et al. 2020, Cottin et al. 2019, Maher et al. 2019). Because these conditions share similarities regarding pathogenesis and clinical behaviour, they are increasingly described under the umbrella terminology of ‘progressive fibrosing ILDs’ or ‘fibrosing ILD with a progressive phenotype (PF-ILD) (Wells et al. 2018). Many ILDs are formally classified as rare (Travis et al. 2013); and PF-ILD is a rare subgroup of ILD patients. Amongst the available literature, PF-ILD prevalence estimates range of 0.22 to 2.00 per 10.000 in Europe and 2.80 per 10.000 in the US. In Belgium PF-ILD prevalence is estimated to be 0.22 per 10.000 (Olson et al. 2018). According to the EMA and FDA definitions, PF-ILD can formally be considered as an orphan disease. In order to complement the data collected in clinical trials, we propose to gather data to describe the diagnosis, disease course and outcomes for PF-ILD patients in Belgium. Idiopathische Lungenfibrose (IPF) ist die häufigste Art der idiopathischen ILD und stellt per Definition den Prototyp der progressiven fibrosierenden ILD (PF-ILD) dar (Raghu et al. 2011). Es ist gekennzeichnet durch eine sich selbst erhaltende progressive Fibrose, eine Verschlechterung der Atemwegssymptome, eine Abnahme der Lungenfunktion, eine Verschlechterung der Lebensqualität und einen frühen Tod trotz klinischer Behandlung. Klinische Daten deuten darauf hin, dass es eine größere Gruppe von Patienten mit unterschiedlichen klinischen ILD-Diagnosen (bekannter und unbekannter Ursache) gibt, die im Verlauf ihrer Erkrankung einen progressiven fibrosierenden Phänotyp entwickeln (Wuyts et al. 2020, Cottin et al. 2019, Maher et al. 2019). Da diese Bedingungen in Bezug auf Pathogenese und klinisches Verhalten Ähnlichkeiten aufweisen, werden sie zunehmend unter der Dachterminologie „progressive fibrosierende ILDs“ oder „fibrosierende ILD mit progressivem Phänotyp (PF-ILD)“ beschrieben (Wells et al. 2018). Viele ILDs werden formal als selten eingestuft (Travis et al. 2013); PF-ILD ist eine seltene Untergruppe von ILD-Patienten. Unter der verfügbaren Literatur liegen die PF-ILD-Prävalenzschätzungen zwischen 0,22 und 2,00 pro 10.000 in Europa und 2,80 pro 10.000 in den USA. In Belgien wird die PF-ILD-Prävalenz auf 0,22 pro 10.000 geschätzt (Olson et al. 2018). Nach den EMA- und FDA-Definitionen kann PF-ILD formal als Orphan Disease betrachtet werden. Omdat deze aandoeningen overeenkomsten vertonen met betrekking tot pathogenese en klinisch gedrag, worden ze steeds vaker beschreven onder de overkoepelende terminologie van ‘progressieve fibroserende ILD’s’ of ‘fibroserende ILD met een progressief fenotype (PF-ILD) (Wells et al. 2018). Veel ILD's zijn formeel geclassificeerd als zeldzaam (Travis et al. 2013); en PF-ILD is een zeldzame subgroep van ILD-patiënten. Onder de beschikbare literatuur, PF-ILD prevalentie schattingen variëren van 0,22 tot 2,00 per 10.000 in Europa en 2,80 per 10.000 in de VS. Als aanvulling op de gegevens die zijn verzameld in klinische onderzoeken, stellen we voor om gegevens te verzamelen om de diagnose, het ziekteverloop en de resultaten voor PF-ILD-patiënten in België te beschrijven. La fibrose pulmonaire idiopathique (FPI) est le type le plus courant de DLI idiopathique et représente, par définition, le prototype de DLI fibrosante progressive (DLI-PF) (Raghu et al. 2011). Elle se caractérise par une fibrose progressive autosuffisante, une aggravation des symptômes respiratoires, une diminution de la fonction pulmonaire, une détérioration de la qualité de vie et une mort précoce malgré la prise en charge clinique. Les données cliniques suggèrent qu'il y a un plus grand groupe de patients avec différents diagnostics cliniques de DPI (de cause connue et inconnue) qui développent un phénotype fibrosant progressif au cours de leur maladie (Wuyts et al. 2020, Cottin et al. 2019, Maher et coll. 2019). Étant donné que ces affections présentent des similitudes en ce qui concerne la pathogenèse et le comportement clinique, elles sont de plus en plus décrites sous la terminologie générique de «LDI à fibrosation progressive» ou de «LDI à fibrosation à phénotype progressif (PF-ILD)» (Wells et al. 2018). De nombreuses DPI sont officiellement classées comme rares (Travis et al. 2013); et PF-ILD est un sous-groupe rare de patients ILD. Parmi la littérature disponible, les estimations de prévalence du PF-ILD varient de 0,22 à 2,00 pour 10 000 en Europe et de 2,80 pour 10 000 aux États-Unis. En Belgique, la prévalence du PF-ILD est estimée à 0,22 pour 10 000 (Olson et al. 2018). Selon les définitions de l'EMA et de la FDA, le PF-ILD peut formellement être considéré comme une maladie orpheline. Afin de compléter les données collectées dans les essais cliniques, nous proposons de collecter des données pour décrire le diagnostic, l'évolution de la maladie et les résultats pour les patients atteints de PF-ILD en Belgique. La modification du remboursement prévoit donc un enregistrement en ligne obligatoire des données médicales et le matériel implanté des implantations de valves cardiaques chirurgicales et aortiques percutanées ainsi qu’un suivi pour les valves aortiques chirurgicales et percutanées (l’encodage des suivis pour les autres valves est facultatif). Cet enregistrement obligatoire permet notamment d’avoir une vue sur toutes les implantations de valves par voie chirurgicales ainsi que sur les implantations de valves aortiques percutanées implantées. L’enregistrement des valves implantées par voie percutanée permet également de collecter des informations sur les indications pour lesquelles il a été décidé de remplacer la valve aortique par voie percutanée plutôt que chirurgicalement. Les deux associations scientifiques concernées, le BWGIC et la BACTS, feront un rapport tous les deux ans sur base des données enregistrées et la présenteront à la Commission." @default.

Momenteel moeten ziekenhuizen voor chirurgische hartkleppen een papieren formulier naar de verzekeraar sturen met enkele gegevens en is er een optionele registratie van percutane aortakleppen via een register bijgehouden door de wetenschappelijke vereniging. De wijziging in de vergoeding voorziet daarom in verplichte online registratie van medische gegevens en geïmplanteerde apparatuur voor implantaties van chirurgische en percutane aortahartkleppen, evenals monitoring van chirurgische en percutane aortakleppen (de codering van monitoring voor andere kleppen is optioneel). Deze verplichte registratie laat in het bijzonder toe zicht te hebben op alle implantaties van chirurgische kleppen alsook op de implantaties van geïmplanteerde percutane aortakleppen. De registratie van percutaan geïmplanteerde kleppen verzamelt ook informatie over de indicaties waarvoor is besloten om de aortaklep percutaan te vervangen in plaats van operatief. De twee betrokken wetenschappelijke verenigingen, BWGIC en BACTS, zullen om de twee jaar verslag uitbrengen op basis van de geregistreerde gegevens en deze aan de Commissie voorleggen." @default.

Derzeit müssen Krankenhäuser für chirurgische Herzklappen ein Papierformular mit einigen Daten an die Versicherungsträger senden, und es gibt eine optionale Registrierung der perkutanen Aortenklappen über ein vom wissenschaftlichen Verband verwaltetes Register. Die Änderung der Erstattung sieht daher eine obligatorische Online-Registrierung der medizinischen Daten und des implantierten Materials der chirurgischen und perkutanen Aortenklappenimplantate sowie eine Nachsorge für chirurgische und perkutane Aortenklappen vor (die Kodierung der Nachsorge für die anderen Klappen ist fakultativ). Diese obligatorische Registrierung ermöglicht insbesondere einen Überblick über alle chirurgisch implantierten Ventile sowie über die implantierten perkutanen Aortenklappen. Die Registrierung der perkutan implantierten Klappen ermöglicht es auch, Informationen über die Indikationen zu sammeln, für die beschlossen wurde, die Aortenklappe perkutan statt chirurgisch zu ersetzen.
